Vasculitis is a clinicopathological
entity that is related to the inflammation of blood vessels and to the
complications caused by the hypoperfusion of end-organs and to the systemic
manifestations caused by immunological disturbances. It is a major challenge
to both rheumatologists and other clinicians since it has a vast symptomatology
affecting nearly every organ system in the body with a high degree of overlap
between the specific syndromes. There has been a substantial progress in
understanding the pathogenetic mechanisms underlying the diseases and in
the classification criteria used to categorise them but the unknowns still
seem to surpass the discovered.
All the authors of this
special issue on vasculitis are members of the Division of Rheumatology
of Cerrahpaşa Medical School, a section with a special interest in vasculitis.
An overview of the vasculitic
syndromes and the problems encountered in the diagnosis and the classification
criteria are reviewed by Prof. Hasan Yazıcı.
The commonly encountered
syndrome of the small vessel vasculitides, the extensive etiological factors
related to it and the major therapeutic approaches in its management are
covered by Dr. İzzet Fresko.
The systemic vasculitides
that affect medium sized vessels such as polyarteritis nodosa and the granulomatous
forms consisting of Wegeners granulomatosis and the Churg Strauss syndrome
are reviewed by Dr. Ayhan Dinç and Dr. İzzet Fresko with special emphasis
to the role of ANCA (anti-neutrophil cytoplasmic antibody) in the diagnosis
and in the pathogenesis of the latter diseases.
The large vessel vasculitides
that are commonly encountered in the young and elderly such as Takayasu
and Temporal arteritis are dealt with in the chapters by Dr. Melike Melikoğlu
Tanverdi and Dr. Pınar Kadıoğlu.
The challenging relationship
between vasculitis and FMF, a disease that is very prevalent in Turkey,
has been analysed by Dr. Cengiz Korkmaz and the latest progress in the
pathogenesis and management of Behçets syndrome has been reviewed by Dr.
The syndromes mimicking
vasculitis, which is an important concept that complicates the differential
diagnosis of the already challenging vasculitic syndromes, is written by
Dr. Melike Melikoğlu Tanverdi.
Finally, the laboratory
aspects and the imaging modalities used in the diagnosis and management
of the vasculitides are scrutinised in the papers written by Dr. Cengiz
Korkmaz and Dr. Ayhan Dinç.
With this collection of
papers on vasculitis, we not only aim to be of some help to the practising
physician, but to attract the interest of the students to the fascinating
and everchanging field of the vasculitides.
